FINANCIAL POLICY
We
request payment for your
initial consultation on the
day of service.
You may contact our
office for our current fees.
After a thorough
examination, we will be able
to establish a fee for
treatment.
Financial
arrangements should be made
before services are
rendered.
If circumstances
beyond your control arise,
we may discuss special
payment arrangements.
Please call or visit
our office to make special
arrangements at least one
full week prior to
treatment.
For
fees of $1000.00 or more,
paid in full on the day of
treatment only, we offer a
5% discount for payment with
cash or check.
This does
not apply to accounts
eligible for network
provider discounts.
We
accept all major credit
cards, as well as Dental Fee
Plan and CareCredit, which
enable you to pay for your
treatment in monthly
installments.
You must notify our
office in advance if you
qualify for Dental Fee Plan
or Care Credit.
As
a courtesy to our patients,
we are happy to file claims
and pre-treatment estimates
with insurance companies.
If requested by your
insurance, we will provide
dental codes, explanations
of benefits, and narrative.
Any other
negotiations must be between
you, as the contract holder,
and your insurance company.
